  • Instructions: Please complete this form, and submit to A1A Transportation, Inc. for Confirmation. Your trip is not confirmed until you receive an e-mail confirming your bus (es).

    • Two days cancellation notice required to avoid minimum payment for the day, which is half the cost of the minimum.
    • Cancellations must be done by fax or e-mail. Phone cancellations will not be accepted.
    • Changes must be done by fax or e-mail.
    • A charge of 1.5% will be applied to invoices over 30 days old.
    • Bus (es) must depart destination at the designated time. Late buses are subject to additional costs.
    • Original invoice will be e-mailed to payment contact person after the field trip is completed.
